Life Experience of Self – Care after Breast Cancer using Modified Radical Mastectomy Method and Caring Radivac Drain Placement at Home: A Phenomenological Case Study

บทนำ: ผู้ป่วยมะเร็งเต้านมที่ได้รับการผ่าตัดแบบถอนรากชนิดปรับปรุงและได้รับการใส่สายท่อระบายกลับบ้าน ผู้ป่วยต้องดูแลตนเองต่อเนื่องที่บ้าน ซึ่งอาจเผชิญผลกระทบในหลายมิติทั้งด้านร่างกายและจิตใจและยังมีงานวิจัยที่ศึกษาประสบการณ์ตรงของผู้ป่วยหลังกลับบ้านค่อนข้างจำกัด
วัตถุประสงค์การวิจัย: เพื่อศึกษาประสบการณ์ชีวิตของผู้ป่วยที่ได้รับการผ่าตัดมะเร็งเต้านมแบบถอนรากชนิดปรับปรุง และได้รับการใส่สายท่อระบายกลับบ้าน
ระเบียบวิธีการวิจัย: การวิจัยเชิงคุณภาพ กรณีศึกษาเชิงปรากฎการณ์วิทยา โดยการสัมภาษณ์เชิงลึกในผู้ป่วยมะเร็งเต้านมที่ได้รับการผ่าตัดด้วยวิธีการผ่าตัดแบบถอนรากชนิดปรับปรุงและได้รับการใส่สายท่อระบายกลับบ้าน ใช้วิธีการสุ่มตัวอย่างแบบบอลล์หิมะ (Snowball sampling) จำนวน 10 คน
ผลการวิจัย: ผลการวิจัย พบประเด็นที่น่าสนใจ 4 ประเด็นหลัก คือ 1)ประสบการณ์ด้านความเครียดและวิตกกังวล 2) การจัดการดูแลตนเอง 3) การได้รับข้อมูล 4) ประสบการณ์การปรับตัวเข้าสู่ยุคปกติวิถีใหม่
สรุปผล: ประเด็นเรื่องการจัดการดูแลตนเอง ภาวะที่ต้องเผชิญ คือ เรื่องสายท่อระบายและการทำแผล ปัจจัยเรื่องค่าใช้จ่ายเป็นส่วนหนึ่งที่ส่งผลต่อการดูแลที่บ้าน การได้รับข้อมูลก่อนกลับบ้านส่งผลต่อชีวิตเพื่อทำให้สามารถผ่านช่วงเวลายุคปกติวิถีใหม่นี้ไปได้
ข้อเสนอแนะ: ประสบการณ์นี้ได้เพิ่มเติมองค์ความรู้ในเรื่องการวางแผนการดูแลตั้งแต่รับใหม่จนถึงการจำหน่ายให้เหมาะสมกับสภาวะการสาธารณสุขของไทยเพื่อให้เกิดความสอดคล้องของการเข้าสู่ยุคปกติวิถีใหม่
