ปัจจัยทางด้านมารดาที่มีผลต่อการเกิดทารกคลอดครบกำหนดน้ำหนักน้อย ในโรงพยาบาลอินทร์บุรี

บทคัดย่อ

ทารกแรกเกิดน้ำหนักน้อย เป็นปัญหาด้านสาธารณสุขที่สำคัญระดับโลก ยังคงมีอุบัติการณ์เพิ่มสูงขึ้น สัมพันธ์กับอัตราตาย และภาวะแทรกซ้อนต่อทารกทั้งในระยะสั้นและระยะยาว การศึกษานี้มีวัตถุประสงค์ เพื่อค้นหาปัจจัยเสี่ยงทางด้านมารดาที่มีผลต่อการเกิดทารกคลอดครบกำหนดน้ำหนักน้อยในโรงพยาบาลอินทร์บุรี เป็นการศึกษาย้อนหลังแบบมีกลุ่มควบคุม เก็บข้อมูลของหญิงตั้งครรภ์ครบกำหนดที่มาคลอดบุตรที่โรงพยาบาลอินทร์บุรีในระหว่างวันที่ 1 มกราคม 2565 ถึง 31 ธันวาคม 2567 จำนวน 320 ราย แบ่งเป็นกลุ่มศึกษา คือ ทารกแรกเกิดน้ำหนักน้อยกว่า 2,500 กรัม จำนวน 80 ราย และกลุ่มควบคุม คือ ทารกแรกเกิดน้ำหนักมากกว่าหรือเท่ากับ 2,500 กรัม จำนวน 240 ราย

ผลการศึกษา พบว่า อุบัติการณ์ทารกคลอดครบกำหนดน้ำหนักน้อย คิดเป็น ร้อยละ 7.63 มารดาส่วนใหญ่มีอายุ 20-34 ปี จบการศึกษาในระดับมัธยมศึกษาหรือ ปวช. และ ประกอบอาชีพรับจ้าง เมื่อพิจารณาปัจจัยของมารดาด้านพฤติกรรมสุขภาพ โรคประจำตัว การฝากครรภ์ และภาวะแทรกซ้อนขณะตั้งครรภ์ นำมาวิเคราะห์ด้วย multiple logistic regression พบ 4 ปัจจัยที่มีผลต่อการเกิดทารกคลอดครบกำหนดน้ำหนักน้อยอย่างมีนัยส้าคัญทางสถิติ ได้แก่ มารดามีภาวะความดันโลหิตสูงขณะตั้งครรภ์ หรือภาวะครรภ์เป็นพิษ (aOR = 11.32, 95% CI: 3.00-42.70) การฝากครรภ์ไม่ครบ 5 ครั้งตามเกณฑ์คุณภาพ
(aOR = 2.50, 95% CI: 1.38-4.52) ลำดับการตั้งครรภ์เป็นครรภ์แรก (aOR = 2.07, 95% CI: 1.18-3.64) และดัชนีมวลกายก่อนการตั้งครรภ์มากกว่าหรือเท่ากับ 30 kg/m2 (aOR = 0.35, 95% CI: 0.13-0.95) ดังนั้น จึงควรพิจารณาปัจจัยเหล่านี้ ร่วมกับการเตรียมความพร้อมของสุขภาพมารดาก่อนการตั้งครรภ์ การประเมินความเสี่ยงระหว่างตั้งครรภ์ และการฝากครรภ์ให้ครบตามเกณฑ์คุณภาพ
