บทคัดย่อ

โรคโพรงกระดูกสันหลังตีบแคบ (lumbar spinal stenosis; LSS) เป็นสาเหตุหลักของอาการปวดหลังส่วนล่าง และข้อจำกัดในการทำกิจกรรมในผู้สูงอายุ การดึงหลังร่วมกับโปรแกรมออกกำลังกายถูกใช้แพร่หลายทางกายภาพบำบัด แต่หลักฐานเชิงเปรียบเทียบระหว่างวิธีออกกำลังกายยังจำกัด

การศึกษานี้ มีวัตถุประสงค์เพื่อเปรียบเทียบผลของโปรแกรม William's Flexion Exercise (WFE) ร่วมกับการดึงหลัง Core Stability Training (CST) ร่วมกับการดึงหลัง และการดึงหลังเพียงอย่างเดียวต่อระดับอาการปวด ความสามารถในการทำกิจกรรม และช่วงการเคลื่อนไหวของกระดูกสันหลังส่วนเอวในผู้ป่วย LSS โดยศึกษาวิจัยเชิงทดลองแบบสุ่ม มีกลุ่มควบคุมเก็บข้อมูลในผู้ป่วยที่ได้รับการวินิจฉัย LSS จำนวน 39 ราย แบ่งเป็น 3 กลุ่ม กลุ่มละ 13 ราย กลุ่มทดลอง คือ กลุ่มโปรแกรม WFE และกลุ่ม CST ได้รับโปรแกรมออกกำลังกายเพิ่มเติมภายใต้การดูแลของนักกายภาพบำบัด ร่วมกับการดึงหลังพร้อมโปรแกรมฝึกที่บ้าน และกลุ่มควบคุม คือ กลุ่มที่ได้รับการดึงหลังเพียงอย่างเดียว

ผลการวิจัย ภายหลัง 4 สัปดาห์ ทุกกลุ่มมีระดับอาการปวด และคะแนนความทุพพลภาพลดลงอย่างมีนัยสำคัญทางสถิติภายในกลุ่ม กลุ่ม CST มีค่าเฉลี่ยของคะแนน Thai Roland–Morris Disability Questionnaire (Thai RMDQ) ต่ำที่สุด (5.92 ± 0.84) เทียบกับกลุ่มโปรแกรม WFE (7.43 ± 0.84) และกลุ่มควบคุม (8.88 ± 0.84) แต่ไม่มีนัยสำคัญทางสถิติ (p = 0.056) การวิเคราะห์ multivariable logistic regression ยืนยันว่ากลุ่ม CST (aOR = 9.27, 95% CI 1.20–71.55, p = 0.033) และกลุ่มโปรแกรม WFE (aOR = 7.97, 95% CI 1.09–58.23, p = 0.041) มีโอกาสเป็นผู้ตอบสนองโดยรวมสูงกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิต
