Problems and Factors Related to Success of Quality in Nursing Records at Emergency Department, Sampran Hospital, Nakhon Pathom Province

Purpose: This study aimed to study problems and success factors related to quality of nursing records in the Emergency Department, Sampran Hospital, Nakhon Pathom Province.
Design: Mixed method research
Method: The purposively selected group was the 10 professional nurses having their work experiences in the Accident and Emergency Department, Sampran Hospital, Nakhon Pathom Province for 2 years and over. This group was assigned to record the nursing data of patients in the nursing documentation. The research instruments included questionnaire to assess factors related to quality of nursing records, the interview questions, and quality of nursing documentation. The patient medical records of 150 patients who had visited to receive the services of accident and emergency at 1-3 levels of severity were evaluated. Data were analyzed and presented into percentage, mean, standard deviation and content analysis.
Main findings: 1) The record was completely done at 78.32%. There were some problems of the non-record data and incompletely recorded data accounted to 15.10 % and 6.58%, respectively. These problems mostly were caused by lack of knowledge, misunderstanding of nursing record related to nursing process, and unawareness of nursing record. Additionally, the problems were associated to their over workloads, rush hours and the other non-nursing related-jobs; 2) For the quality of nursing documentation, it was found that the complete information included patient general data (98.57%), and the patient first admission information (95.67%). While the nursing diagnosis was completely recorded only a few amounts (27.42%); 3) The successful factors namely: the predisposing factor (knowledge and understanding of nurses), the enabling factor (infrastructures and facilities) and the reinforcing factor (the supporting of organization), they were all at high level.
Conclusion and recommendation: The results of this research show that nursing administrators should develop and enhance all three factors such as enhancing knowledge, providing manuals and regular verification of nursing documents.
