The effect of the development of a service model for One Day Surgery in cervical biopsy with Loop Electrosurgical Excision Procedure

Abstract

One Day Surgery (ODS) is a quality improvement approach in health service provision. This research aimed to compare the effects of the ODS service model (experimental group, n = 64) with inpatient service (control group, n = 64) for patients undergoing Loop Electrosurgical Excision Procedure (LEEP) on (1) pain levels, (2) the cost of care, and (3) patient satisfaction with ODS. A two-group quasi-experimental design was applied. Participants were purposively selected from patients undergoing LEEP at the Department of Obstetrics and Gynecology, Chiang Kham Hospital, Thailand. The research process consisted of four steps: (1) problem identification, (2) planning, (3) implementation, and (4) evaluation. Data collection tools included a service record form, a pain assessment form, and a satisfaction questionnaire. Data were analyzed using descriptive statistics (frequency, percentage, mean, and standard deviation). Since data were not normally distributed, the Mann-Whitney U test was used to examine differences in pain scores.

The results showed that: (1) Pain levels at the time of procedure, 2 hours, 24 hours, and beyond 24 hours post-procedure showed no statistically significant difference between the two groups (p > 0.05), (2) The ODS service model reduced the cost of care by 10.55%, and (3) Patient satisfaction with ODS service was at the highest level. ODS care was delivered through a multidisciplinary team approach, involving patients and relatives, with post-discharge follow-up conducted via telemedicine using the “Mor Prom” application.
