Spiritual Well-being, Social Support and Perspective of Advance Care Planning among People with Chronic Disease and Relatives

Abstract

Purpose: This research aimed to examine spiritual well-being, social support and perception of advance care planning among people with chronic disease, and to study the relationship between spiritual well-being, social support and perspectives of advance care planning among people with chronic disease, as well as, to study perspectives of advance care planning among the relatives of people with chronic disease.

Design: Descriptive correlational research

Method: The sample comprised of 1) 87 people who were diagnosed with chronic diseases and 2) Ninety-one relatives of patients with chronic diseases who were admitted to the hospitals.  Purposive sampling was used to recruit the sample. The instruments used were demographic information questionnaire, the Functional Assessment of Chronic Illness Therapy- Spiritual Well-Being, FACIT-Sp-12 questionnaire, the social support questionnaire, and the advance care planning opinion questionnaire for people with chronic disease and the relatives. Data were analyzed using descriptive statistics and Spearman rho correlation.  

Main Findings: 1) People with chronic diseases showed a high level of spiritual well-being and social support, perception of advance care planning in terms of no prolonging life was high.            2) There was small positive relationship between social support and spiritual well-being, as well as,  advance care planning in terms of no prolonging life (r = 0.24, 0.22, p-value < 0.05 respectively) But there was no relationship between spiritual well-being and advance care planning in terms of no prolonging life (r = 0.05, p > 0.05). 3) The relative perception of advance care planning in terms of no prolonging life was high.

Conclusion and recommendations: Nurses should promote spiritual well-being and social support for people with chronic illnesses, and integrate communication regards to advance care planning, especially while patients have decision-making capacity. This includes facilitating family involvement to support shared decision-making.
