Abstract

     A 40 year old Thai male patient had an accident from riding a motorcycle into a dog and then was admitted to the hospital on May 30, 2020 by the delivery of foundation. The patient cannot remember the event. The patient’s head is swollen on the forehead and the right ear and both nostrils of patient are bleeding. The patient was admitted in the hospital for 232 days and then was discharged and delivered continuous care at home for follow-up visits. It was found that the patient had a complex disease that required special care after discharge. The patient was inserted a urinary catheter. The visit was planned into 3 phases: pre-visit phase, visit phase and end-visit phase. Four follow-up visits were conducted. Caregivers were able to care for the patient without no recurrence of treatment. From the study, it was found that continuous follow-up and assessment of problems and needs throughout the time of care makes it possible to plan care to help patients and caregivers in accordance with their needs completely.
