Abstract

     Objective To examine the development and outcome assessment of the continuing care models in patients with schizophrenia by caregivers and network partners in the community, Huaiyot, Trang Province.

     Materials and methods This research and development further develop and build-on the development of psychiatric network which using C3TRER. This research is the new look by using 7 steps of mental health department guidance as a development framework, 48 patients with schizophrenia were sampled leading to selection criteria. Using the follow-up form to collected Data. Global Assessment of Functioning (GAF) scale and WHO Quality of Life – BREF (WHOQOL - BREF) were applied. Wilcoxon signed ranks test and Pair samples t-test were analyzed.

     Results The research showed a significant reducing in patients with severe recurrence which referred according to service plan. The continuing care showed that significant different in severity of symptom, medication adherence, daily life routine, working ability, family relationship, score of GAF and WHOQOL - BREF before and after the development.

     Conclusion Based on the research and development showed that reducing relapses and severe recurrence, and improve ability of function and quality of life.
