Nursing care of Ischemic stroke patients during Rehabilitation in Kham muang Hospital: 2 Case Study

This study was aimed to compare nursing care for patients with ischemic stroke during rehabilitation period. This two cases study of rehabilitation patients with stroke by purposive selection , Which Entered for treatment in the ward of Kham Muang Hospital between January - March 2023, amounting to 2 cases.The data were collected from medical record ,interviewed patients anf their relative. The functional health pattern of Gordon's 11 Concepts of Health Model was used to assess health problems to planning a holistic nursing care , nursing diagnosis, nursing care planning program ,and to evaluate the outcome of nursing care of rehabilitation patient with stroke.
The results of the study found that case study 1:A 69-year-old Thai male patient with left limb weakness was diagnosis of Right MCA infraction. The patient was refer back from Kalasin Hospital by disharge and then back to stay at home for treatment ,which patient had a history of comorbidities, diabetes, high blood pressure. The patient had Glasgow coma scale were E4VTM6 had left limb weakness, left motor power grade O, right grade V. The throat of patient was pierced and on techeostomy tube, and eating food through a tube. While being treated at a community hospital, There were a complication difficulty coughing up phlegm having stiff joints and pressure sores on the hip area. After receiving treatment for complications, the patients' symptoms gradually improved. and can be sold back home. Case study 2: A 63-year-old Thai female patient with Stroke track S/P rt- PA diagnosis. The patient was refer back from Kalasin Hospital to come to rehabilitate at a hospital near the house. The patient had significant symptoms weak left arm, crooked mouth, stiff tongue, slurred speech. After receiving rt-PA anticoagulant therapy and stay at the hospital for 7 days, The patient was referred for treatment at a nearby hospital. History comorbidities of patient that were diabetes, high blood pressure. Glasgow coma scale E4VTM6 left arm and leg weakness, motor power left, grade III right, grade V right. It was treated with medication and rehabilitation. Patients were able to perform their activities and their symptoms improved accordingly. no disability left Available to go home.
