Abstract

     This research was participatory action research aimed to study the situation and self-management problems of diabetic patients. To develop and evaluate the effectiveness of a self-management model in behavior modification of diabetic patients on glycemic control by community participation. The sample group was diabetic patients. Thirty-five diabetic patients with uncontrolled blood glucose levels were calculated from the sample size. The results were evaluated using a behavioral questionnaire. The accumulated sugar level in the blood Analyze the data with Descriptive statistics Inferential statistics paired t-test.

     The results of the study found that the self-care behavior promotion program with community participation to control sugar for diabetic patients at Nong Han Hospital, Nong Han District, Udon Thani Province, consisting of 1) participation: create community participation, 2) Information: support information, knowledge, and 3) Nonghan self-management: self-management model. The results of self-management model evaluation (Nonghan self-management) found that the behaviors in 5 aspects are food self-management; exercise Emotional side of drug taking side of smoking and drinking alcohol at a good-high level, all and found that food self-management Exercise, mood and blood sugar levels Different from before the model development was statistically significant at the significance level of 0.05.
