Developing a System of Care by Sepsis Alert Team for Patients with Severe Sepsis in Secondary Care Hospital

Abstract

     The action research aimed to develop service system management for severe sepsis patients by Sepsis alert Team and study the result of the development of service system management .The method used was an action research consisting of the following 6 Building Blocks of A health System uring October 2020 – September 2022 .Evaluation.the result after develop service system management for severe sepsis patients admit in emergency room and ward at secondary care hospital 1,119.case .Data were analyzed by descriptive statistics using frequency,percentage and chi-square test Analyze qualitative data using content analysis.

     The results was showed that A health System,2021,2022 early severe sepsis diagnosis 97.20%,95.37% fluid with or without vasopressors ,fluid for hypotension/ hyperlactatemia for achieve of surviving sepsis campaign bundle targets 97.76%,99.66%,Blood culture before antibiotic 96.27%,98.97%. All of that results was found to have increase. The mortality rate was found to have decrease by 0.13%, 9.39,9.26 For achieve of surviving sepsis for severe sepsis patient level 2-3 in intensive care unit untired management in 3 hour that results was found to have increase 50.00%,80.00%.Develop service system management of severe sepsis in patient admitted at secondary care hospital compliant early severe sepsis diagnosis, fluid with or without vasopressors, fluid for hypotension/ hyperlactatemia for achieve of surviving sepsis campaign bundle targets, Blood culture before antibiotic, Broad spectrum antibiotics within 3 hours for emergency and 1 hour for others, surviving sepsis for severe sepsis patient level 2-3 in intensive care unit untired management in 3 hour that results was found significant(p-value < 0.05).This study shows the existence of the development of service system using 6 Building Blocks of A health System in this study might fully representative achieve of surviving sepsis campaign bundle targets for severe sepsis management and improve effectiveness of service system and accessibility and quality of service .
