Nursing care of Ischemic Stroke patients with Complication during Rehabilitation in Kalasin Hospital : A Study

Abstract

     Comparative study of 2 case studies, purposively selected among ischemic stroke patients who received treatment at Kalasin Hospital between September - October 2023 by collecting medical history data from doctors. Records from history taking of patients and relatives using Gordon's 11 health patterns. Orem's theory of self-care to find problems and plan nursing actions, summarize and continuously evaluate nursing results.

     The results of the study found that the first case study was a Thai female patient, age 35 years, with weakness in the right arm and leg, diagnosed with Ischemic Stroke, admitted for treatment at the Home Ward of Kalasin Hospital. Continue to rehabilitate your body. Treatment plan: Continue taking medication to control high blood pressure. History of co-morbidities high blood pressure Continued follow-up care: Glasgow coma scale (GCS) E4VTM6 had symptoms of weakness in the left arm and leg. Motor power on the left was grade V, right was grade III. Complications were found to be high blood pressure. Difficulty moving Impaired self-care and economic problems After receiving continuous treatment, the patient's condition gradually improved.

     Case study number 2: Thai female patient, age 77 years, diagnosed with Ischemic Stroke after treatment with rt-PA clot-dissolving drug. Complications occurred and treatment was brain surgery: Craniectomy with remove clot. Patient refused. congenital disease The doctor sent him home for rehabilitation. Continue to follow up and take care of GCS E4V5M6. There is weakness in the left arm and leg. Motor power on the left is grade I, right is grade V. Receive medication and rehabilitation. Experience swallowing problems movement impaired self-care After treatment and rehabilitation, the patient's overall condition improved accordingly.
