การใช้ยา Meropenem ในโรงพยาบาลพระนารายณ์มหาราช

การศึกษานี้เป็นการศึกษาแบบย้อนหลังมีวัตถุประสงค์เพื่อศึกษาการใช้ยา Meropenem ความเหมาะสมการสั่งใช้ยาผลการรักษาของยา Meropenem และปัจจัยที่สัมพันธ์กับการสั่งใช้ยา Meropenem ที่ไม่เหมาะสมในโรงพยาบาลพระนารายณ์มหาราช โดยศึกษา ระหว่างวันที่ 1 ตุลาคม พ.ศ. 2561 – 30 กันยายน พ.ศ. 2565 กลุ่มตัวอย่าง คือ ผู้ป่วยที่มารับการรักษาแบบผู้ป่วยใน โรงพยาบาลพระนารายณ์มหาราชที่ได้รับยา meropenem จำนวน 350 คน เครื่องมือที่ใช้ในการวิจัยเป็นแบบบันทึกข้อมูล วิเคราะห์ข้อมูลโดยใช้สถิติพรรณนา (Descriptive statistics) และการวิเคราะห์ถดถอยลอจิสติก (Multiple logistic regression)
ผลการวิจัย : จากจำนวนกลุ่มตัวอย่าง 350 คน โดยได้รับยาในแผนกอายุรกรรมจำนวน 236 คน (ร้อยละ 67.43) แผนกศัลยกรรมจำนวน 114 คน (ร้อยละ 32.57) ซึ่งลักษณะของผู้ป่วยทั้งสองกลุ่มส่วนใหญ่ไม่มีความแตกต่างกัน ยกเว้น อายุของผู้ป่วยในแผนกศัลยกรรมที่อายุ 15-45 ปี การใส่ท่อช่วยหายใจ และการติดเชื้อที่เกี่ยวข้องกับชุมชน มีสัดส่วนมากกว่าแผนกอายุรกรรม ผู้ที่ได้รับการสั่งใช้ยา Meropenem ไม่เหมาะสมตามข้อบ่งชี้จำนวน 53 คน (ร้อยละ 15.15) เมื่อวิเคราะห์ปัจจัยร่วมทำนาย พบว่า การรับส่งตัวมารักษา (refer in), ผู้ป่วยใส่ท่อช่วยหายใจ และรูปแบบการรักษาแบบ Empirical treatment มีความสัมพันธ์กับการสั่งใช้ยา Meropenem ที่ไม่เหมาะสมในโรงพยาบาล พระนารายณ์มหาราชอย่างมีนัยสำคัญทางสถิติ (p<0.05)
