The Development of Advance Care Plan Model in Palliative Care at Khokpho Hospital.

Abstract

     This research and development aimed to study the conditions and problems in palliative care for terminally ill patients at Khok Pho Hospital, Pattani Province, to develop a self-care plan in advance for palliative care for terminally ill patients, and to evaluate the effectiveness of the self-care plan in advance for palliative care for terminally ill patients. The sample consisted of 30 patients and relatives who were caring for terminally ill patients at Khok Pho Hospital and Subdistrict Health Promotion Hospital, Khok Pho District, Pattani Province, who were diagnosed by doctors as patients who needed palliative care. Data were collected from questionnaires and assessment forms. Data were analyzed using content analysis, frequency, percentage, mean, standard deviation, and Dependent t-test.
     The results of the evaluation of the results of the use of the model found that the comparison of the results of the sample group's practice in initiating a conversation in palliative care for terminally ill patients using the POS assessment form (for patients) before and after receiving care in each case showed a statistically significant improvement (P<.001). The results of the evaluation of the satisfaction of patients and relatives who received palliative care were at a very satisfactory level (M= 2.73, SD=0.45). The average satisfaction of the model development team was also found to be very satisfactory (M= 3.00, SD=0.00). In addition, comparing the number of days of stay and medical expenses in the group receiving care according to the advance care planning model with the group not receiving advance care planning, it was found that the group receiving advance care planning had fewer days of stay and medical expenses.
