Long-Term Care Model for Dependent Elderly with Community Participation in a Subdistrict Health Promoting Hospital in Southern Thailand

The development of long-term care models for dependent elderly necessitates a participatory design to align with the changing demographics of an aging society. This action research study aimed to develop a participatory long-term care model for dependent elderly in a sub-district health promotion hospital in Southern Thailand. The study comprised four stages: 1) planning, 2) action, 3) observation, and 4) reflection. Key informants included 1) 34 dependent elderly, 2) 32 nursing personnel, and 3) 8 caregivers of the dependent elderly. Data were analyzed using descriptive statistics, inferential statistics (Paired-samples t-test), and content analysis.
The research findings revealed that the participatory long-term care model for dependent elderly consists of three main components: 1) structural component, which are resources and systems necessary for efficient service provision, with 4 sub-components: caregiver personnels, agencies support, close relatives available, and information communicate; 2) component for providing home healthcare and rehabilitation services, which are mechanisms for participatory and continuous long-term care for the dependent elderly, with 3 sub-components: access and continuity of care, relatives and caregivers participation, and specialized care; and 3) component of long-term care outcomes, which are health outcomes, mental and social outcomes, burden reduction of relatives, and satisfaction. Assessing the Activities of Daily Living (ADL), the average score after implementing the care model increased by 0.79 compared to before its implementation (95% CI: 0.30-1.27), indicating statistical significance.
