Nursing care of postpartum hemorrhage with hypovolemic shock with total hysterectomy: case study

Abstract

This study aimed to study nursing care of postpartum hemorrhage with hypovolemic
shock with total hysterectomy.
Results: A case study of a 25 year old Thai woman who was pregnant for the first
time. The maternal with symptoms after childbirth was admitted to the postpartum ward on
September 28, 2021. The maternal had 750 cc of vaginal bleeding, low blood pressure of
80/50 mm Hg and abnormal blood coagulation. Doctor diagnosed that the maternal had
postpartum hemorrhage and disseminated intravascular coagulation. Maternal was received
explored laparotomy with total and nursing care from the beginning until taking care of
entering the operating room. The maternal had no complications from the hypovolemic
shock. After that maternal was transferred to intensive care unit for 2 days. After surgery,
maternal was still a risk of recurrent shock and had anemia. Therefore maternal was received
intravenous infusion and blood, and drug administration until safety. In recovery period,
maternal had pain and risk of wound infection and then maternal was received pain therapy and proper wound care. The maternal had no infection and recovered accordingly. The
maternal was discharged on October 4, 2021, total 6 days of hospital administration. After
follow up, the maternal had no complications.
