Development of Chronic Care System for patients with chronic Kidney Disease state 3-4 of Non Sung Hospital

Abstract

The purpose of this research was to develop a care system for patients with Chronic
kidney disease (CKD). Study of the results of the development of the Chronic kidney disease
state3-4 care system developed using Case management System Theory . By evaluating
clinical results And the quality of life divided into 2 phases ; The first phases , Developing a
care system for patients with.CKD state3-4 Sample is Multidisciplinary care for kidney disease
patients 14 case, Non Sung Hospital, 2 doctors, 11 nurses, 1 nutritionist, and 30 new case of
CKD state3-4. Data were collected by using Semi-structural interview and tape recording
Measurement of results from the development of a new care system for the newly
developed chronic kidney disease. Data were analyzed using content analysis. Get a system
model using Case management System Theory Concept . It consists of defining duties and
responsibilities that Clear of the multidisciplinary team Guidelines for taking care of kidney
disease state3-4 patients Nursing care in modern kidney disease state3-4 patients Patient care
outcome indicators and a continuous holistic care system Phase 2 ; The effect of developing
a care system for Chronic kidney disease state3-4 patients using a conceptual model based
on Case management System Theory developed from the first phase. Select the sample
group, specifically 30 new case diagnosed chronic kidney disease state3-4 patients out of 30
patients admitted to the CKD Clinic of Non Sung Hospital .Providing care by using a model to
develop a care system for chronic kidney disead state3-4 patients for 6 months. Data were
collected before and after the experiment by using clinical outcome and quality of life
assessments. Data were analyzed using descriptive and paired t-test statistic.
The result of the research shows that Patients have clinical outcomes, including Kidney
