Development of an Integrated Home-Based Care Model for Dependent Older Adults through Home Visits and Holistic Nursing at Ban Siri Phatthana Health Promoting Hospital, Nam Om Subdistrict, Kho Wang District, Yasothon Province

This study employed a research and development (R&D) design with the objectives of developing an integrated home-based care model for dependent older adults that combines home visits and holistic nursing care, and evaluating the effectiveness of the developed model. The study was conducted at Ban Siripattana Health Promoting Hospital, Nam Om Subdistrict, Kho Wang District, Yasothon Province, Thailand. The sample consisted of 46 dependent older adults. The research instruments included the integrated home-based care model for dependent older adults combining home visits and holistic nursing care, an Activities of Daily Living (ADL) assessment, a Quality of Life (QOL) assessment, and a satisfaction questionnaire regarding the care model. Data were analyzed using descriptive statistics including frequency, percentage, mean, and standard deviation. The Wilcoxon Signed-Rank Test was used to compare outcomes before and after the implementation of the model.
The results showed that after the implementation of the developed care model, dependent older adults had significantly higher scores in Activities of Daily Living and Quality of Life compared to those before the implementation at the .05 level of statistical significance. In addition, the overall satisfaction of older adults with the care model was at the highest level. The developed care model promoted comprehensive care for older adults covering physical, psychological, and social aspects, and supported the participation of families and communities in caring for older adults. The findings of this study can serve as a guideline for developing community-based care systems for dependent older adults and for supporting appropriate health service delivery within the local context.
