Development of Stroke Patients Care Model in Srisangwornsukhothai Hospital

Abstract

This research and development aimed to develop and evaluate the stroke patient care model in Srisangwornsukhothai hospital. Samples were stroke patients, primary caregivers, and a multidisciplinary team. The development process was divided into four steps including 1) situation analysis, 2) development of the care model, 3) implementation of the model, and 4) evaluation of the model. Research instruments were composed of the stroke patient care model, clinical outcome record, a test about stroke care for caregivers, and a satisfaction questionnaire for the multidisciplinary team. Data were analyzed with frequency, percentage mean, standard deviation, independent t-test, and Mann-Whitney U test.
The study results showed that patients in the stroke care model were composed of a stroke care unit, multidisciplinary teams, a nurse case manager, a stroke care plan, and a clinical practice guideline. After using the stroke patient care model, patients had more abilities to perform daily activities than those before using the stroke patient care model both at the date of discharge and after 2 weeks (p=.02. p=.02), had less length of hospital stay than those before using the stroke patient care model (p=.03). However, pneumonia, urinary tract infection, and pressure sore incident rates between the two groups were not significant. The caregivers’ knowledge about stroke significantly increased (p-value= .001). The overall satisfaction level of the multidisciplinary team is at high level. Therefore, this model should be used in taking of these patients. The healthcare team should continue to evaluate patients’ quality of life and promote their care.
