The development of nursing system for patients with multiple trauma in Buddhachinaraj Phitsanulok Hospital
Abstract
This research and development study aimed to develop a nursing care system for patients with multiple trauma. The study was conducted in four phases: 1) situation analysis, 2) system development, 3) implementation of the nursing care system, and 4) system evaluation. The sample consisted of two groups: 1) 29 registered nurses and 2) 84 patients, with 42 patients in both the control and intervention groups. The research instrument was the development of a nursing system for patients with multiple trauma. Data collection tools included: 1) personal data record forms, 2) focus group interview guidelines, 3) the nursing care system, 4) nurse assessment tools—knowledge test, nursing practice evaluation, competency assessment, and satisfaction questionnaire, 5) patient satisfaction questionnaire, and 6) patient outcome record forms. Data were analyzed using frequency, percentage, mean, standard deviation, chi-square test, and paired t-test.
The results revealed three key issues in the nursing care system: 1) staffing, 2) care processes, and 3) support systems. These issues led to the development of the “SSO System,” which includes 1) staffing management processes, 2) nursing service organization, and 3) outcome monitoring and improvement processes. After implementing the system, nurses’ mean scores for knowledge and practice significantly increased (p < .001). Their overall competency improved compared to before the system implementation, and they reported a high level of satisfaction with the system. Additionally, patients were assessed and triaged within 10 seconds, shock was evaluated and managed within 4 minutes, and pain management was provided within 60 seconds — all of which showed statistically significant improvement (p < .001).
Recommendation: The “SSO System” has been shown to systematically and safely enhance the quality of patient care. It is therefore recommended that this system be promoted and implemented in other healthcare settings.
