Development of unofficial-time nursing supervision model in Buddhachinaraj Phitsanulok Hospital

This research and development study aimed to develop and evaluate the implementation of an after-hours nursing supervision model at Buddhachinaraj Phitsanulok Hospital. The research was divided into three phases: 1) problem analysis, 2) development of the after-hours nursing supervision model, and 3) implementation and outcome evaluation of the model. The sample consisted of registered nurses divided into two groups: 120 team leader nurses and 200 registered nurses working after-hours in the following departments: medical intensive care, surgical, pediatric, orthopedic, and obstetrics-gynecology. Qualitative data were analyzed using content analysis, while quantitative data were analyzed using frequency, percentage, mean, and standard deviation. Data comparisons were conducted using a paired t-test and a Chi-square test.
The results revealed that the developed model comprised three components: 1) knowledge provision through training workshops, 2) standard practice adherence via nursing performance supervision, and 3) support and encouragement. The development outcomes showed that the team leader nurses' knowledge of nursing supervision significantly increased after implementing the model. The team leader nurses' overall satisfaction with the model was at a high level, and the registered nurses' overall satisfaction with the team leaders' supervision was at the highest level. Regarding adherence to practice guidelines, the findings indicated that after implementing the model, practices for preventing pressure ulcers, falls, and medication administration improved. Concurrently, the incidence of pressure ulcers, falls, and medication errors decreased with statistical significance (p-value < 0.05).
The recommendation for implementing the research findings is that this after-hours nursing supervision model should be utilized to empower team leader nurses to provide appropriate supervision. This enables the supervisees to perform nursing practices efficiently, ultimately leading to the continuous development of high-quality nursing services.
