The effectiveness of the preparedness program for knee arthroplasty at Thabo Crown Prince Hospital

This two-group quasi-experimental study aimed to evaluate the effectiveness of a preparedness program for patients undergoing total knee arthroplasty at Thabo Crown Prince Hospital. The sample included 32 patients with knee osteoarthritis scheduled for surgery, divided equally into an experimental group and a control group (n = 16 each), along with 25 professional nurses. Research instruments consisted of the preparedness program, a demographic questionnaire, postoperative assessment forms (Modified WOMAC scale and Knee Flexion record), a knowledge questionnaire, and satisfaction scales. The instruments demonstrated an Index of Item-Objective Congruence (IOC) between 0.67–1.00 and a Kuder-Richardson 20 (KR-20) reliability coefficient of 0.81. Data analysis was performed using percentages, independent t-tests, and one-sample t-tests.
The results revealed that the experimental group achieved significantly higher mean scores for self-care knowledge and satisfaction compared to the control group. Additionally, the experimental group showed significantly lower mean pain scores (Modified WOMAC scale) and a higher rate of knee flexion exceeding 90 degrees than the control group (p-value < .05). Regarding the professional nurses, their satisfaction across all five dimensions was significantly higher than the standard criterion (3.00) (p-value < .05).
Recommendation: The preparedness program proved effective in enhancing both the knowledge and physical performance of patients. It is recommended that this program be integrated into standard nursing care, spanning from the preoperative phase in the orthopedic clinic to post-discharge follow-up. Emphasis should be placed on providing essential information, training self-care skills, and offering psychosocial support in collaboration with community health networks to facilitate patient adaptation and confidence in returning to their daily activities.
