Accidents involving the operation of privately owned trucks represent a significant occupational safety concern, particularly for organizations with off-site work activities. This study aimed to assess the probability of accident occurrence resulting from unsafe conditions in a utility and facility management company in Thailand. A cross-sectional analytical design was employed, integrating the Analytic Hierarchy Process (AHP) and Fault Tree Analysis (FTA) to identify root causes. The study involved a total of 12 participants. These included five specialists in safety, engineering, or science, and two members of the company’s Occupational Safety and Health Committee, who provided judgments for constructing the fault tree. In addition, five specialists conducted pairwise comparisons to determine the relative importance of contributing factors. Data were analyzed using Expert Choice Version 11 and Reliability, Availability, Maintainability, and Safety (RAMS) Software Version 8.5 to estimate event probabilities. The findings revealed that improper loading practices were the most influential cause of accidents (65.25%), followed by unsafe road conditions (65.02%) and vehicle unpreparedness (64.67%). These results underscored the critical role of physical and unsafe conditions in contributing to accident risk. Therefore, organizations should implement systematic and proactive risk management strategies to mitigate accident risks and promote a sustainable occupational safety culture.
