Abstract

Hepatitis C virus (HCV) is a primary cause of liver cirrhosis and hepatocellular carcinoma (HCC). Treatment with sofosbuvir/velpatasvir is associated with reduced mortality rates, decreased incidence of liver failure, and a decline in liver-related complications, including HCC. The objective of this study was to evaluated the effectiveness of sofosbuvir/velpatasvir with or without ribavirin for chronic hepatitis C treatment at the hepatitis clinic, Udon Thani hospital. This study was a retrospective descriptive study. Data were collected from HCV patients aged 18–70 years at the hepatitis clinic of Udon Thani hospital between December 7, 2024, and December 31, 2025. A total of 100 patients were enrolled and divided into two groups: those receiving sofosbuvir/velpatasvir (n = 50) and those receiving sofosbuvir/velpatasvir + ribavirin (n = 50). Statistical analyses included descriptive statistics, Fisher's exact test, and Mann-Whitney U test. Treatment effectiveness was measured by the sustained virological response at 12 weeks post-treatment (SVR12). The study included 100 patients with mean age of 54.26±12.77 years. Of these, 55 patients were male (55.00 %), 37 patients had cirrhosis (37.00%), and 22 patients were co-infected with HIV (22.00%). All patients were treatment-naïve. The overall SVR12 rate were cure 97 from 100 patients (97.00%). Sofosbuvir/velpatasvir group from 49 patients were achieved SVR12 (98.00%). Sofosbuvir/velpatasvir + ribavirin group from 48 patients were achieved SVR12 (96.00%). The SVR12 rate was 90.90% (20/22) in patients with HIV co-infection and 94.59% (35/37) in patients with cirrhosis. There was no statistically significant difference in the SVR12 rate between the cirrhotic and non-cirrhotic groups (p-value = 0.315). Furthermore, among the cured group, the Fibrosis-4 (FIB-4) index decreased, although this reduction was not statistically significant. Adverse event, minor anemia and fatigue were observed in the ribavirin group; however, 100.00% of patients were able to complete the full course of treatment. Therefore, both regimens sofosbuvir/velpatasvir alone and in combination with ribavirin demonstrated high effectiveness (98.00% and 96.00%, respectively) and demonstrates a strong safety profile for all subgroups of HCV patients within the real-world clinical context of Udon Thani hospital that suitable and correlation with Thai public health policy “Test and Treat HCV elemination 2030”
