ประสิทธิผลของรูปแบบการดูแลแผลกดทับในชุมชนสำหรับผู้ป่วยติดเตียง โดยคลินิกหมอครอบครัว อำเภอท่าปลา จังหวัดอุตรดิตถ์

การศึกษานี้มีวัตถุประสงค์เพื่อประเมินประสิทธิผลของรูปแบบการดูแลแผลกดทับในชุมชน ภายใต้การดำเนินงานของ คลินิกหมอครอบครัว อำเภอท่าปลา จังหวัดอุตรดิตถ์ โดยเป็นการวิจัยกึ่งทดลองในผู้ป่วยติดเตียง 30 รายที่มีแผลกดทับระดับ 2–3 รูปแบบการดูแลประกอบด้วย แนวทาง Wound Bed Preparation (WBP) ควบคู่กับ การฝึกสอนและเสริมศักยภาพผู้ดูแล และการติดตามผลผ่านระบบ Telemedicine กระบวนการหายของแผล ประเมินโดยใช้แบบ PUSH Tool Version 3.0 และเฝ้าระวังภาวะแผลกดทับติดเชื้อในสัปดาห์ที่ 2 และ 4 การวิเคราะห์ข้อมูลใช้สถิติ Paired sample t-test เพื่อเปรียบเทียบค่าเฉลี่ยของคะแนน PUSH และใช้ McNemar's test เพื่อวิเคราะห์การเปลี่ยนแปลงของจำนวนผู้ป่วยที่มีภาวะแผลกดทับติดเชื้อ ก่อนและหลังการใช้รูปแบบการดูแล
ผลการศึกษา พบว่า คะแนน PUSH ลดลงอย่างมีนัยสำคัญทางสถิติ (p < 0.001) โดยค่าเฉลี่ยของคะแนนลดลงจาก 12.26 ± 2.67 เป็น 10.43 ± 3.26 ในสัปดาห์ที่ 2 และ 8.73 ± 4.44 ในสัปดาห์ที่ 4 และพบภาวะแผลกดทับติดเชื้อลดลงจากร้อยละ 80.0 เป็นร้อยละ 6.7 ในสัปดาห์ที่ 2 และร้อยละ 10.0 ในสัปดาห์ที่ 4 (p < 0.001)
