Development of a medical treatment model for diabetic patients in the corona virus 2019 epidemic situation, case study in Rattanaburi Hospital, Surin Province

Abstract

     This research and development aimed to develop and study the effectiveness of a medical treatment model for diabetic patients in the situation of the COVID-19 outbreak of Rattanaburi Hospital, Surin Province. There are 2 groups of research participants, consisted of 15 public health personnel group and related network partners and 48 diabetic patients. The research was conducted during July 2022 - March 2023. Data were collected by focus group, questionnaires and blood glucose levels recording. Data analysis was performed by descriptive statistics, paired T-test, with level of significance at 0.05 and content analysis. The results showed that the medical treatment model for diabetic patients in the situation of the COVID-19 outbreak of Rattanaburi Hospital, there are 1) diabetic patient care models in health care facilities. Patients were grouped by the level of disease control measures. Screening method and providing treatment for patients as a one-stop service. Social distancing was used in non-communicable disease (NCD) clinic intensely, improve the system for refilling medicines by doctors. 2) Making the service system connection by applying 3 doctors policy and a health station. The 1st doctor, village health volunteers (VHVs) to take care of diabetes patients in the green group. Promote the use of primary screening services in the community. The 2nd doctor (public health personnel) takes care of diabetes patients in the yellow group. The third doctor (physician) takes care of the patients in the red group and complex groups, provide services at the NCD clinic at the hospital. All groups of diabetic patients were followed up by staff of public health personnel, and VHVs, emphasizing on delivering medicines to patients at home. 3) Giving advice and follow up on behavior use both on the phone and the LINE application. The results of using the developed model showed that diabetic patients had an increase in overall health behavior and a statistically significant decrease in blood sugar levels at the 0.05 level, and were very satisfied with the developed model at a high level. Therefore, this developed diabetic patient care model can be used in the treatment of diabetic patients effectively under the situation of the COVID-19 outbreak.
