Abstract

     This research and development aimed to study the situation of stroke and develop a care model for stroke patients who receive mid-term care. The sample group was purposively selected to be new ischemic stroke patients within the first 6 months. The total number of patients was 34. The study period was from December 2019 to December 2020. The data collection tool included focus
group questions. Brainstorming record form In-depth interview Practical skills assessment form for caregivers and volunteers, ADL assessment form, complication assessment form, and satisfaction assessment form. Data were analyzed using content analysis. Descriptive statistics and comparing results before and after using the model using t-test statistics.
     The results of the study found that volunteerism in the community and interdisciplinary professions Have correct knowledge, understanding and skills Stroke patients who received mid -term care had higher scores in their ability to perform daily activities than before the development (p<0.05). Symptoms improved by 91.17%. Complications of pressure sores occurred in 1 case, accounting for 1 case. 2.94% were assessed and diagnosed as disabled at the end of 6 months, 2 cases accounting for 5.88%. The main caregivers, volunteers and the multidisciplinary team were at the highest level of satisfaction with the care model.
