Abstract

     This Research was participatory action research aimed to study the development and outcome assessment of a proactive care model for schizophrenia patients, integrated with primary care cluster services and community networks. Sample size were 96 people. The research instruments including a demographic questionnaire, Basic of psychotic assessment, Global Assessment of Functioning (GAF) scale and WHO Quality of Life-BREF (WHOQOL-BREF), Satisfaction assessment were used for data collection. Data were analyzed by using frequency, percentage, mean, Wilcoxon signed ranks test and Pair samples t test.

     The research results found that revealed that the development of a proactive care model for schizophrenia patients, integrated with primary care cluster services and community networks had 3 stage, 1) Need analysis and conceptual framework 2) Prototype development 3) Action and outcome evaluation, before and after the development of care model finding that the Basic of psychotic assessment, GAF scale, Quality of Life were significantly at .05 level (p < .01)
