Abstract

     This research was research and development aimed at developing and evaluating a system of care for end-stage chronic kidney disease patients who are not receiving palliative renal replacement therapy with the participation of a multidisciplinary team and affiliates. net Research period: April 2021 to June 2022, 69 participants in the study. Research tools consisted of group discussion questions. In-depth interview, observation form, AAR assessment form, patient assessment form Patient care program satisfaction assessment form Patient care manual Qualitative data were analyzed using content analysis and quantitative data were analyzed by number, percentage, mean, and comparing the results of the activities with t-test statistics.

     The results showed that All patients received advance care plan. Shortness of breath was reduced by 75%, confusion was reduced by 50%, and death peacefully at home was 94.74%. The average score of palliative care results according to patient perception and according to the perception of the caregiver's relatives statistically significantly decreased than before the experiment Mean scores at the end of life in peace based on patients' perceptions. and according to the perception of relatives, carers were higher than before the experiment was statistically significant Patients and caregivers have a high level of satisfaction.
