Abstract

     This research was research and development. The objective was to study the context and health conditions of the elderly. and the development of a health care operation model for the elderly in Pak Phli District, Nakhon Nayok Province. The study period is from October 2020 to September 2022. The study period is divided into 2 phases: Phase 1 (Research) prototype study. Implementation model of elderly health care in the area of Ban Nong Hua Ling Nai Health Promoting Hospital, Nong Saeng Sub-district, Pak Phli District, Nakhon Nayok Province Study from October 2020 to September 2021, sample size were 410 elderly. Phase 2 (Development) The expansion of the implementation model for health care for the elderly in Pak Phli District, Nakhon Nayok Province. Study from October 2021 to September 2022. Sample size were 5,059 elderly collected data from assessment and measurement forms. data analysis by Content analysis, frequency, percentage, mean, standard deviation, Dependent t – test and F – test by one way ANOVA technique.

     The results of the study showed that Phase 1 (Research) was a prototype study. Implementation model of elderly health care in the area of Ban Nong Hua Ling Nai Health Promoting Hospital, Nong Saeng Sub-district, Pak Phli District, Nakhon Nayok Province Study in October 2020 to September 2021. Health knowledge of the elderly Practice behaviors "Don't fall, don't forget, don't get depressed, eat delicious food" or 4 Smart before and opinions about the 5-dimensional happiness model after the action. overall and side at the highest level and There was a statistically significant difference at the .05 level. higher than before operation. Phase 2 (Development) The expansion of the implementation model for health care for the elderly in Pak Phli District, Nakhon Nayok Province. Study in October 2021 to September 2022. After the implementation, the elderly at home (ADL score between 5 - 11) decreased from 7.80% to 3.04% and the elderly stuck at home (ADL score between 0 - 4) decreased from 1.15% to 0.89%. and health knowledge of the elderly Practice behaviors "Don't fall, don't forget, don't get depressed, eat delicious food" or 4 Smart, and opinions about the 5-dimensional happiness model after the operation. Overall and each aspect was at the highest level, with a statistically significant difference at the .05 level. higher than before operation
