Abstract

     This research is an action research to develop a model for caring for patients with bloodstream infections Pla Pak Hospital Nakhon Phanom Province. The purpose of this study was to develop and study the effects of the development of a care model for patients with bloodstream infections. Pla Pak Hospital Nakhon Phanom Province. The study was conducted between May and July 2023. The samples used in the research were: 1. The health team consisted of physicians, professional nurses, Medical Technologist Radiologists, pharmacists, nutritionists, 20 people. 2) Patients with sepsis who receive services at Pla Pak Hospital Nakhon Phanom Province, 30 people, comparing before and after. The research tools consisted of The tools used for data collection were general information questionnaires. a satisfaction assessment form and a feasibility assessment form of using the care model for patients with sepsis Pla Pak Hospital Nakhon Phanom Province. The instruments used in the experiment were a model and a manual for the care of patients with bloodstream infections. Caring for patients with bloodstream infections Pla Pak Hospital Nakhon Phanom Province. Data analysis Descriptive statistics were used to find frequency, percentage, mean and standard deviation. The satisfaction scores were analyzed before and after the development using Pair t-test, the level of statistical significance was set at 0.05.

     The results of the study revealed that 1) the care model for patients with bloodstream infections Pla Pak Hospital Nakhon Phanom Province consisted of (1) Screening for sepsis in sub-district promotion hospitals using qSOFA (2) Hospital screening (Triage) into Sepsis fast track (3) Treatment plan in the urgent phase (4) Septic shock nursing, (5) monitoring signs and symptoms of severe infection, (6) recording time of diagnosis and recording time of physician examination, and (7) planning discharge. bloodstream infection patients Pla Pak Hospital Nakhon Phanom Province found that (1) the average satisfaction of the health team It was found that after the system development, the mean satisfaction increased from before the system development significantly 0.05 ( gif.latex?\bar{x}=22.45, S.D.= 3.12; gif.latex?\bar{x}=33.65, S.D.= 1.98) and patient satisfaction. It was found that after the system development, the mean satisfaction was significantly increased than before the system development by 0.05 (gif.latex?\bar{x}=21.86, SD = 3.27; gif.latex?\bar{x}=33.70, SD = 2.13) (2). of using the patient care model for bloodstream infections Pla Pak Hospital Nakhon Phanom Province of the overall health team It was found that after developing the system, there was a consensus on the possibility of using a model to care for patients with bloodstream infections. Pla Pak Hospital Nakhon Phanom Province, 100 percent, and when classifying the findings, it was found that There is consensus on the possibility of using a model of care for patients with sepsis.
