Abstract

     This study was case study aimed to study the nursing care for total knee arthroplasty with adrenal insufficiency post surgery Result :A 63-year old Thai woman was admitted to Pakchong Nana Hospital with major symptom pain in both knees difficulty walking 1 year before coming to the hospital Doctor diagnoses Osteoarthritis Of the Knee treated by taking painkillers from the clinic. Sometimes she buy medicine to take herself and take various dietary supplements together with physical therapy Symptoms do not subside the doctor made an appointment for surgery to bilateral total knee arthroplasty she was admitted to the hospital on May 30,2023 in the preoperative period found a problem with knee pain patients and relatives are worried about surgery during surgery, blood loss during surgery was 50 cc. During surgery, 170 cc of blood drains were removed from both sides. she had low blood pressure from adrenal insufficiency (stress cortisol =1.1). Treated in the initial period with Levophed IV drip and Hydrocortisone 100 mg IV bolus. then 200 mg +5%D/W 100 ml IV drip in 24 hrs* 2 day patient safe Then received post-operative nursing care. Monitoring for other complications The patient is safe, has been rehabilitated to the point where he can walk with a walker and can be discharged home on June 5, 2023.
