Results of the discharge planning program for patients with ischemic stroke or blockage at Nong Khai Hospital.

This quasi-experimental research aimed to compare the number of bed days and readmissions to hospital within 28 days and compare the knowledge of caregivers before and after using the discharge planning program for patients with vascular disease. The brain and readiness of the caregiver Nong Khai Hospital The sample group has 60 people and must have a caregiver by purposive selection. Data collection began during October 2023 - December 2023. There are 2 sets of tools used for data collection: patient and caregiver personal information forms. and the knowledge test of the caregiver Reliability was checked using the KR 21 formula and was equal to 0.9. Data were analyzed using percentage, mean, standard deviation, t-test (t-test).
The results showed that the average knowledge of caregivers within the experimental group and between groups Both before and after using the discharge planning program, it was found that the experimental group's knowledge increased significantly more than the control group at the .05 level. Hospitalization of stroke patients after using the discharge planning program Plan for discharge according to guidelines for care for stroke patients in the experimental group. The number decreased significantly more than the control group at the .05 level.
