Nursing care for patient with hypertension diabetes and congestive heart failure : a case study

Abstract

     This study was case study aimed to study Nursing care for patient with hypertension and diabetes with heart failure. By study a 42-years-old Thai female patient came to the outpatient department on 3 January 2023 with symptoms of shortness of breath, swollen legs on both sides, and little urine output 1 day before coming to the hospital. The patient had a history of high blood pressure for 4 years and diabetes and had tracheostomy for a lung infection for 2 years.
     Results: Chest x-ray results showed fluid in the right pleural space (Pleural Effusion Rt). The patient was diagnosed as congestive heart failure. The nurse provided nursing care before the examination. Patient had a risk of tissue oxygen depletion, high blood pressure and concerned about symptoms and existing diseases. During the examination, the patient's breathing efficiency decreased. The patient had a risk of danger from high blood sugar levels, abnormal mineral salts in the body and a decrease in kidney function. After the examination,the patient is worried about the disease, not enough knowledge, not behaving properly. Patients received nursing care as planned at every stage and was sent for treatment to the female medical ward. The patient was discharged on 7 January 2023, totaling 4 days of treatment. From following treatment, checking and receiving medicine according to appointment, the patient had no complications.
