This research was action research aimed to study the situation in order to develop a model for continuing care for dependent elderly people. and evaluate development results sample size were 30 pairs of dependent elderly patients and their caregivers, conducting research from July 2023 - April 2024. Tools for conducting research Including a semi-structured questionnaire. Assessment of team satisfaction with model development Assessment of daily activities (Activity Daily Living, ADL) and assessment of home health care behavior of caregivers and nurse satisfaction Statistics used in data analysis include frequency, percentage, mean, and standard deviation. and inferential statistics, including Pair t-test statistics.
The results of the research found that: After using the model, it was found that the mean satisfaction of the team was at a satisfactory level (M=3.56, SD=0.76). The mean score of health care behavior of elderly patients and home caregivers was at a good level (M= 2.56, SD=0.52) and comparing the mean score of ability to perform daily activities before and after increased with statistical significance (t=-5.12, p<.05)
