Development of a care model for patients with uncontrolled hypertension by applying social support in the area of Krapho Subdistrict Health Promotion Hospital,Tha Tum District, Surin Province.

Abstract

     This study was an action research (AR) to develop a care model for patients with uncontrolled hypertension by applying social support in the area of Krapho Subdistrict Health Promoting Hospital, Tha Tum District, Surin Province, based on the concept of Kemmis and McTaggart. Two purposive sample groups were selected: 1) a sample group for collecting qualitative data and developing a care model for patients with uncontrolled hypertension, purposively selecting 12 patients; and 2) 35 patients with uncontrolled hypertension. They participated in the research process at Krapho Subdistrict Health Promoting Hospital between March - May 2025. Quantitative and qualitative data were collected from questionnaires and focus group discussions. Quantitative data were analyzed using descriptive statistics, and blood pressure levels and inferential statistics were analyzed using Paired-T test and Wilcoxon Signed-Rank Test. The developed model consisted of four activities: 1) activities to understand patient and family problems; 2) activities to design a care plan for patients, families, communities, and public health personnel together; 3) ongoing operations and support, especially family members, as role models and facilitators in behavior change; and 4) follow-up, evaluation, and appreciation.
