Nursing care of a pregnancy with pregestational diabetes complicated by shouder dystocia : case study.

Abstract

     This study was case study aimed study the nursing care of a laboring woman with pregestational diabetes mellitus complicated by shoulder dystocia.One case of a woman with pregestational diabetes mellitus and shoulder dystocia during labor was selected. Nursing care was provided based on the nursing process.

     Results: The case involved a 22-year-old pregnant woman, G2P1D1L0, at 39 weeks of gestation, diagnosed with pregestational diabetes mellitus. She was admitted to Phra Narai Maharaj Hospital on October 9, 2024, for planned delivery. During the antepartum period, she was at risk for both hyperglycemia and hypoglycemia, and the fetus was at risk for fetal distress and shoulder dystocia. During labor, shoulder dystocia occurred and lasted for 4 minutes. Nursing care included preparation and assistance during labor, pain relief, and neonatal support immediately after birth. The mother remained safe, and the newborn was transferred to the neonatal intensive care unit for further monitoring. During the postpartum period, care was focused on preventing postpartum hemorrhage and teaching the mother how to express breast milk for the baby. On the third postpartum day, both mother and baby had no complications. Breast milk production was sufficient, and the mother was able to breastfeed successfully. The physician discharged both mother and baby home after a 3-day hospital stay.
