Development of Rational Drug Use for the kidney in the community at Samdet District, Kalasin Province.

This research was action research. The objectives are to study drug use problems among kidney patients and at-risk groups, the use of "Know Your Kidney" colored stickers, drug use problems in patients receiving treatment in the hospital and home visits, and rational drug knowledge about kidneys in the community. The study is divided into 6 phases. The sample group includes kidney patients and at-risk groups receiving services at the NCD clinic at Somdej Hospital who received services between 1 September and 30 September 2025. Data were analyzed using descriptive statistics.
The results found drug use problems in 51 kidney patients and risk groups, the majority did not know the stage of kidney, but know that their kidney was good or bad is 74.50%. Dietary control to prevent kidney disease is 15.68%. Amount of Drinking water according to kidney stage is 56.86%, taking other medications affect the kidneys by using NSAIDs, herbal medicines is 15.69% and Know about of herbal medicines affect the kidneys is 62.74%. A study is use colored stickers of Kidney stage in 112 kidney disease patients found that the average eGFR was 46.59±8.79 ml/min/1.73 m2. There were 84, 24 and 4 patients with kidney stage 3a, 3b and 4 respectively. 46.43% perceived good/bad kidney stage, 21.43% know about of behavioral adjustments that could slow kidney failure, 53.57% know about of dietary adjustments to prevent kidney failure, 35.71% know about of drinking water appropriately according to kidney stage and 25.00% know about of contraindications medications affect the kidneys by using NSAIDs, herbal medicines in kidney patients. After providing education, it was found that patients knew more about kidneys. As for the study of the problem of inappropriate drug use in patients receiving treatment at the hospital, it was found 6 incidence: NSAIDs caused AKI 2 incidence, combination drugs caused AKI only one incidence and Hepatitis only one, too. Herbs caused AKI 1 incidence, and allergic reaction to antimicrobial drugs 1 incidence. The inappropriate drug use in home visiting, there were 5 incidence: 2 patients bought combination drugs and 3 stroke patients bought dietary supplements. Knowledge of kidney drugs in the community, 18 village health volunteers were provided knowledge. It was found that knowledge about chronic kidney disease before and after providing knowledge had an average score of 80.26±2.12 and 90.79±2.12, respectively. Then, 85 people in the community were questioned. It was found that as for health literacy The average knowledge was 85.44±10.79 , divided into knowledge about accessing health information and health services at 78%, communication skills at 64.29%t, decision-making skills at 73.24%, media literacy at 82.7%, and self-management at 84.35%.
