The Development of Continuing Care Model for Patient with Acute Ischemic Stroke in Selaphum hospital Roi-et Province

This research and development aimed to objective was to develop a continuous care
model for patients with Acute Ischemic Stroke at Selaphum hospital, Roi Et province. The
purposively selected samples comprised of 35 patients with Acute Ischemic Stroke at Selaphum
hospital. Research instruments include the general questionnaire, the Barthel ADL Index of Patients'Daily Activities Assessment Form of Neurology Institute, Department of Medical Services, Ministry of
Public Health. The Modified Rankin Scale (mRs) and assess the satisfaction of patients and their
care giver.
The results showed that: After adopting a continuing care model in acute ischemic stroke
patients It was found that on the date of discharge of the patient from the hospital, the average
scores on the ability to perform daily activities increased more than the date of admission. The
patients were hospitalized with statistical significance (p-value = <0.0001, 95%CI = 1.94 – 3.36). The
mean disability assessment score (The Modified Rankin Scale : mRs) was significantly lower than the
date of admission to the hospital (p-value = <0.0001, 95%CI = 0.44 – 0.81. ) And the satisfaction of
patients and their care giver found that the average score of overall satisfaction with the service
was 4.11 (sd = 0.79. ) is at the highest level.
