This research was action research aimed to study the situation of self-care and service system for diabetic patients in the Phutthamonthon Hospital service network. Nakhon Pathom Province. The study period is between October 2020 and September 2021, totaling 12 months. Sample size were diabetic patients in the Phutthamonthon Hospital Service Network in 2020, namely Khlong Yong Sub-district, 2,308 cases, Maha Sawat Sub-district, 2,308 cases, and Salaya Sub-district, 4,591 cases, a total of 8,702 cases, in 2021; in Khlong Yong Subdistrict, 1,363 cases, Maha Sawat Subdistrict 1,572 cases, and Salaya Subdistrict 3,182 cases, a total of 6,117 cases. Collect data from medical record form and questionnaire of health behaviors in self-care of diabetic patients who come to service network of Phutthamonthon Hospital. Analyze the data by Content analysis, Frequency, Percentage, Mean, Standard Deviation, Dependent t-test and F-test by One way ANOVA technique pairwise comparison by Duncan and F-test by repeated measurement technique pairwise comparison by LSD
The results of the study after arranging the service system for diabetes patients in the Phutthamonthon Hospital service network. Under the COVID-19 situation, it was found that Health behaviors of diabetic patients who received services at the Phutthamonthon Hospital service network in 2021, Khlong Yong Subdistrict, Maha Sawat District, Salaya and all sub-districts. who received services at the Phutthamonthon Hospital service network in 2021 found that overall and each aspect at the very much level. And sugar levels in diabetic patients who received services at the Phutthamonthon Hospital service network in 2021, found that normal 60.3%.
