Development of elderly care model using local wisdom for chronic disease management at Nonsrisawat Sub-district Health Promoting Hospital, Namon District, Kalasin Province.

This participatory action research aimed to develop an elderly care model integrating local wisdom for chronic disease management at Ban Nonsrisawat Sub-district Health Promoting Hospital, Namon District, Kalasin Province. The study was conducted from July 2525 to February 2026, spanning 8 months. Qualitative samples comprised 50 participants including health personnel, village health volunteers, local wisdom practitioners, community leaders, and family caregivers for model development. Quantitative samples included 70 elderly with chronic diseases for model evaluation. Research tools included focus group discussion guidelines, knowledge and self-care behavior questionnaires, quality of life assessment forms, and satisfaction questionnaires. Quantitative data were analyzed using descriptive statistics and inferential statistics (Paired t-test), while qualitative data were analyzed using content analysis.
Results revealed that the developed care model comprised nine main components called ACCESS CARE Model: Assessment, Collaborative Care, Continuity of Care, Activities based on Local Wisdom, Reliable Local Wisdom, Educational Enhancement, Economic Empowerment, Emotional & Mental Health Care, and Support System. After model implementation, knowledge scores increased from 8.20 ± 2.10 to 12.80 ± 1.40, self-care behavior scores improved from 39.04 ± 8.23 to 61.23 ± 6.47, and quality of life scores increased from 39.59 ± 8.47 to 59.73 ± 6.28, all statistically significant at the .05 level (t = -18.42, -19.87, -17.65 respectively). The developed model enabled elderly to improve their knowledge, behavior, and quality of life, with high satisfaction.
