DEVELOPMENT OF AN ASSESSMENT MODEL FOR CHRONIC KIDNEY DISEASE PATIENTS TO ENTER PALLIATIVE CARE AT DONMODDAENG HOSPITAL, UBON RATCHATHANI PROVINCE

This quasi experimental research aimed to: (1) develop a rapid and comprehensive assessment model for end-stage chronic kidney disease (CKD) patients to enter palliative care, and (2) examine the effects of the model on patients’ and families’ perceptions of disease progression and their quality of life. The sample consisted of 30 patients with stage 4 and 5 CKD receiving services at the non-communicable disease clinic of Donmoddaeng Hospital, selected through purposive sampling. Research instruments included: (1) a researcher-developed assessment model for screening patients for palliative care,
(2) a disease progression perception questionnaire, and (3) the WHOQOL-BREF-THAI quality of life assessment tool. Data were collected before and three months after the implementation of the model. Data were analyzed using descriptive statistics and paired-samples t-test.
The results showed that the developed assessment model was practical for clinical use, with nurses reporting a high level of satisfaction (Mean = 4.65, SD = 0.48). After the implementation, patients’ and families’ scores for perception of disease progression and palliative care significantly increased (p < .001). Furthermore, the patients’ overall quality of life scores significantly improved from a mean of 58.75 (SD = 10.24) to 76.50 (SD = 8.91) (p < .001).
