The development of self-management promotion model for type 2 diabetes patients in diabetes school of Si-Leam sub-district, Prakhon Chai district, Buriram province

This action research aimed to study the development of self-management promotion model for type 2 diabetes patients in diabetes school of Si-Leam sub-district, Prakhon Chai district, Buriram province. The samples consisted of 121 type 2 diabetic patients, selected based on predetermined criteria. Quantitative data was collected by questionnaire, personal behavior and health Status record and focus group discussion guidelines. Data was collected between October 2025 to March 2026. Data analysis was performed by descriptive and comparative analysis was performed by paired t-test with a significance level of 0.05. Qualitative data using content analysis.
The research findings showed the developed model comprised 5 components: 1) Self-Management Education and self-assessment (12-week learning-based program) 2) Action Planning 3) Reinforcement 4) Capacity Building and 5) Monitoring and Evaluation. The evaluation results showed a comparison of pre and post development of a self-management promotion model found the mean score difference of knowledge and self-management behaviors were significantly higher than pre-development and was significantly reduction in HbA1c levels (p-value<0.001).
