Incidence and clinical predictors of acute pulmonary embolism in patients undergoing CTPA, King Narai Hospital

This survey research by retrospective study aimed to determine the incidence, associated factors, and predictive factors of acute pulmonary embolism (APE) among patients undergoing computed tomography pulmonary angiography (CTPA) at King Narai Maharaj Hospital. A total of 270 patients were included, with data collected from medical records. The research instrument was a structured data collection form developed from literature review, demonstrating excellent content validity (IOC = 1.00) and high reliability (Cronbach’s alpha = 0.92). Data were analyzed using descriptive statistics, univariate analysis, and multivariable logistic regression, with statistical significance set at p < 0.05.
The results showed that the incidence of APE was 28.1%. Most patients were classified as having moderate clinical probability according to the Wells criteria. Factors significantly associated with APE included syncope, cardiac arrest, obesity, immobilization, estrogen use, elevated D-dimer levels, abnormal echocardiographic findings, and a Wells score ≥ 4. Multivariable logistic regression analysis identified syncope, cardiac arrest, obesity, immobilization, estrogen use, abnormal echocardiographic findings, and Wells score ≥ 4 as independent predictors of APE with statistical significance.
