Development of a Fast-Track System for Screening and Referral of Patients with Sepsis at the Outpatient Department of Mahasarakham Hospital.

This study was a Research and Development (R&D) aimed at developing a screening and referral model for patients with sepsis using a fast-track system in the outpatient department of Mahasarakham Hospital. The study was conducted in four phases: Phase 1, situational and contextual analysis of the existing screening and referral system for sepsis patients; Phase 2, development of the screening and referral model; Phase 3, implementation of the developed model; and Phase 4, evaluation of the model’s effectiveness. Data collection instruments included semi-structured interviews, a model suitability assessment form, and questionnaires. Data were analyzed using descriptive statistics, including frequency, percentage, mean, and standard deviation, as well as inferential statistics using paired t-test.
The results revealed that in Phase 1, patient screening lacked standardized guidelines, with personnel relying primarily on clinical experience. The referral system was delayed and lacked continuity. In Phase 2, the developed model consisted of four components: (1) screening, (2) decision-support system, (3) fast-track referral, and (4) continuity of care, incorporating SIRS, qSOFA, and S-NEWS criteria. The content validity index was 0.89. In Phase 3, implementation results showed that personnel achieved complete screening coverage, with high levels of accuracy in documentation and application of screening criteria. In Phase 4, the model significantly reduced screening time, decision-making time for referral, and time to transfer patients to the emergency department (p < 0.001). Personnel satisfaction with the model was at the highest level (mean = 4.64).
