Development of a Fast Track Emergency Surgery Model for Acute Appendicitis Patients to Reduce Waiting Time and Postoperative Complications in a Hub Hospital.

Abstract

      This study was Research and Development (R&D) aimed to develop and evaluate a Fast Track emergency surgery model for acute appendicitis patients within the Borabue–Kudrang–Nachueak health service network. A study was conducted from August 2024 to October 2025, applying two PDSA quality improvement cycles across four phases: situation analysis, model development, implementation, and evaluation. Participants included acute appendicitis patients treated within the network and multidisciplinary healthcare personnel, selected by purposive sampling.

      Results: The developed "BORABUE Fast Track Model" comprises seven components: Baseline standardization using the Modified Alvarado Score, Optimized referral system, Rapid triage and diagnosis, Accelerated operating room access, Bridging network collaboration, Unified multidisciplinary team, and Evaluation with KPIs. Following implementation, 80.70% of eligible patients entered the Fast Track pathway. Mean door-to-operation time decreased from 10.8 to 5.6 hours (48.1% reduction). The perforation rate declined from 18.45% to 8.54%, surgical site infection rate from 9.52% to 3.57%, and mean length of stay from 3.9 to 2.1 days. Average treatment cost per patient decreased by 5,300 THB. No mortality or 28-day readmissions were recorded. Patient and staff satisfaction scores were high (Mean = 4.58 and 4.35, respectively).
