The results of case management for patients with uncontrolled diabetes using brief advice and brief intervention techniques Kosumpisai hospital.

Abstract

      This research was a quasi-experimental research using a one-group pretest-posttest design, integrated into routine work at the Chronic Disease Clinic, Outpatient Department, Kosumpisai Hospital. The objective was to examine the effects of brief counseling on self-care behaviors and glycated hemoglobin (HbA1c) levels among diabetic patients with uncontrolled blood glucose. The sample consisted of 50 participants selected according to inclusion criteria and voluntary participation. Data were collected using the Diabetes Self-Care Behavior Assessment Form developed by the Health Education Division (2019).

      Of the participants, 14 (28%) were male and 36 (72%) were female, with a mean age of 52 years. The most common occupation was government officer (47.56%), followed by farmer (36.25%) and self-employed/other (11.30%). Among them, 45 (90%) were outpatients and 5 (10%) were inpatients. After participating in the program, 44 participants (88%) showed a decrease in HbA1c levels, 1 participant (2%) had no change, and 5 participants (10%) showed an increase. Post-intervention, participants demonstrated significantly higher self-care behavior scores and a significant reduction in HbA1c levels (p < 0.05). Additionally, 94.8% of participants had motivation scores ≥8, corresponding to the decision-making and action stages of behavioral change.
