Nursing care of stroke patients in recovery phase at community.

Abstract

     This case study was analysis with academic principles and applies the results of the study to the care of patients with rehabilitative stroke in the community. Aimed to study the course of the disease. Evaluation and diagnosis of medical treatment and outcomes Nursing In Nursing Stroke Patients in the Rehabilitation Phase in the Community.      Study results : This patient had an acute stroke with underlying diseases such as HT, DLD, CKD, and had symptoms of stiff tongue, slurred speech, weakness in the right limb, and underwent treatment for 12 days. Received 7 home visits, found nursing problems, including the pre-home visit. Relatives of caregivers are worried due to lack of knowledge about the disease and rehabilitation guidelines. Nursing problems were found, patients had impaired physical movements. Unable to meet the needs of daily routine due to weakness in the right hemisphere of the body. Patients are at risk of falling into bed accidents because they are blind in their right eye and cannot help themselves. The patient is at risk of infection in the system, impaired communication due to difficulty speaking. Patients are at risk of dehydration and food because they cannot eat on their own, and patients are at risk of developing pressure ulcers due to reduced body movement. In the period after the home visit, the patient was at risk of recurrence  of stroke, advising relatives to observe the symptoms of brain changes that require the patient to be sent for treatment, such as crooked mouth, slurred speech, increased limb and leg weakness, and recommending an emergency number 1669.
