Developing a secure patient identification system for surgical procedures to prevent errors in the operating room at Borabue Hospital.

Abstract

      This action research aimed to develop and evaluate a safe patient identification model for surgical procedures to prevent operating room errors. The sample group consisted of 17 operating room personnel and 30 surgical patients. The research was conducted between March and August 2026. The research was carried out in five stages: problem analysis, model design and development, two pilot tests of the model, model refinement, and model implementation. The tools used included a knowledge assessment form, a practice assessment form, a patient identification record form, and a satisfaction assessment form. Data were analyzed using frequency, percentage, mean, standard deviation, and content analysis.

      The results showed that after the model development, personnel's knowledge scores increased from a mean of 6.47 (SD = 0.72) to 9.17 (SD = 0.63), and their practice scores increased from 6.94 (SD = 0.96) to 9.05 (SD = 0.96), with statistically significant differences at the .05 level. The developed model is called the C4 Model. After the model was implemented, no incidents of wrong patient, wrong side, or wrong site surgery were detected. Personnel expressed the highest level of satisfaction with the overall model.
