Abstract

This Research and Development (R&D) project aimed to study the situation, develop, and evaluate a nursing model for patients with sepsis at Phayakkhaphum Phisai Hospital. The study was conducted in four phases between October 2023 and September 2024. The sample group consisted of 30 sepsis patients who received the model based on Tanner's Clinical Decision-Making Theory, Orem's Self-Care Theory, and Meleis' Transition Theory, combined with Donabedian's Framework for Evaluating Structure, Process, and Outcomes. A control group of 42 sepsis patients were considered before the implementation of the model. Instruments were knowledge test and the satisfaction questionnaire. Data were analysed using descriptive statistics, paired t-test, independent t-test, chi-square test, and content analysis.

The study produced the PHAYAK Model, comprising six components: Prevention and awareness building, Hunting for cases proactively, Activating a sepsis fast track, Yielding targeted surveillance, Allying the team and family, and Keeping continuity of care. After implementation, nurses' knowledge and nursing practice scores increased significantly (p < .001). Compliance with the one-hour sepsis bundle rose from 59.52% to 90.00% (p = .004), door-to-antibiotic time decreased by 35.81 minutes (p < .001), and the average length of stay decreased by 2.21 days (p = .010). Septic shock incidence and 28-day readmission showed a downward trend that did not reach statistical significance. Nurses' overall satisfaction with the model was at the highest level.
